Committee Purpose

The Analytical Committee advances analytical science in therapeutic drug monitoring (TDM) by providing practical support, scientific guidance and education to scientists, laboratory professionals, pharmacists and clinicians involved in TDM.

  • Support the development, validation, troubleshooting and implementation of HPLC, LC–MS/MS and other analytical methods, including the transition from immunoassays to chromatographic and mass spectrometric techniques.
  • Develop practical guidance for routine laboratories, complementing rather than replacing existing ICH, FDA, EMA and ISO 15189 requirements and relevant metrological and traceability frameworks, including those associated with the Joint Committee for Traceability in Laboratory Medicine (JCTLM).
  • Promote high-quality analysis using conventional and alternative sampling strategies, including dried blood spots (DBS), volumetric absorptive microsampling (VAMS) and other microsampling approaches.
  • Provide regular, globally accessible educational activities, with particular support for early-career professionals, new laboratories and laboratories in resource-limited settings.
  • Work across IATDMCT committees and regional sections to provide analytical expertise in assay development, validation, harmonisation and clinical implementation.

The Committee adapts its priorities to members’ interests, emerging analytical technologies and the evolving needs of TDM practice.

Practical guidance and scientific publications

  • Free-drug fraction measurement: an ongoing joint consensus initiative with the IATDMCT Anti-Infective Drugs Committee, led by Prof. Véronique Stove, addressing analytical standardisation and methodological considerations for unbound drug measurements.
  • Microsampling-based method standardisation: an ongoing position/consensus paper led by Prof. Tomasz Pawiński and Dr Arkadiusz Kocur. The project covers DBS, VAMS and related approaches, including pre-analytical factors, assay validation, analytical performance and clinical implementation. A possible systematic assessment of validation practices in published microsampling studies has also been proposed.
  • Drug measurement in breast milk: an ongoing manuscript led by Prof. Jan-Willem Alffenaar. The literature search has been completed and data extraction is in progress. Current work examines analytical assay validation and breast-milk matrix variability across the day and postpartum period. Two Pharmacy Honours students are contributing; their findings are intended to be consolidated into a manuscript for expert and Committee input.

Educational activities and webinars

  • 21 May 2026 — Joint webinar with the Anti-Infective Drugs Committee: “Analytical Science Driving Better Anti-Infective Therapy: Challenges, Innovations, and Clinical Translation.”
  • 29 April 2026 — Dr Josko Ivica contributed “Troubleshooting Assays for TDM & Analytical Committee Contributions” to the IATDMCT webinar “TDM with Limited Resources – The Way Forward.”
  • Planned for late November/early December 2026 (tentative) — webinar on “Analytical Challenges in Alternative Sampling Strategies.”
  • Topics under consideration for future webinars and workshops include transitioning from HPLC-UV to LC–MS/MS, method development and validation, laboratory implementation, analytical troubleshooting, and analytical performance specifications.

Member engagement and collaboration
Following the hybrid Committee meeting in Rennes on 28 September 2026, the Chair and Vice-Chair launched an internal member expertise initiative. Members are invited to share their analytical and clinical expertise, research interests, methodologies and areas in which they wish to contribute. The aim is to identify complementary expertise, support focused working groups, facilitate collaborations and create opportunities for colleagues at different career stages.
The Committee also supports analytical capacity building through cooperation with IATDMCT regional activities, including initiatives supporting TDM laboratory development in Indonesia.

Strategic priorities for 2027–2028
Topics proposed for further development include analytical performance specifications for TDM; harmonisation of terminology and reporting; reference methods, metrological traceability and external quality assessment; non-targeted drug analysis and surveillance; automation and high-throughput LC–MS/MS; alternative matrices and decentralised sampling; AI- and data-driven analytical approaches; and educational resources for laboratories implementing LC–MS/MS. These areas represent proposed priorities rather than formally launched projects.

2026

24th IATDMCT Congress, Rennes, France (27–30 September 2026)

  • Symposium 7: “Innovations in Drug Analysis: Non-Targeted Approaches, Challenges, and Clinical Implications.”

2025

23rd IATDMCT Congress, Singapore (21–24 September 2025)

  • Symposium 4: Simplifying Drug Assays for Low-Resource Environments
  • Symposium 7: Creative Solutions for Global Challenges in Fentanyl Monitoring and Toxicology

The Committee is exploring potential analytical contributions and cross-committee scientific activities for the 2027 IATDMCT Congress in Montreux.

  • Scientific contributions to the IATDMCT Congress programmes in Singapore (2025) and Rennes (2026), including sessions focused on low-resource drug assays, fentanyl monitoring and non-targeted analytical approaches.
  • Webinars and educational presentations addressing anti-infective TDM, method troubleshooting and analytical practice in resource-limited environments (April and May 2026).
  • Ongoing development of scientific guidance and manuscripts concerning free-drug fraction measurements, microsampling method standardisation and breast-milk drug analysis.
  • Committee meetings supporting project coordination and member participation: online meeting on 17 February 2026, and hybrid meeting at the 2026 Rennes Congress on 28 September 2026 (approximately 20 attendees in person and online).
  • Engagement with other IATDMCT committees to identify potential shared publications, workshops and research initiatives.
  • Kocur A. “Analytical Challenges in Therapeutic Drug Monitoring of Selected Antifungals Using Alternative Sampling Strategies: Emphasizing Microsampling.” COMPASS, December 2025, Volume 24, Issue 4.
  • Li H, Yang L, Delaney S, Colantonio D, Stefan C, Ivica J. “Antiepileptic Drugs in Canadian Clinical Laboratories.” COMPASS, March 2026, Volume 25, Issue 1.
  • Moczulski M. “Analytical Challenges in Therapeutic Drug Monitoring of Selected Antifungals Using Alternative Sampling Strategies: Emphasizing Microsampling.” COMPASS, September 2026, Volume 25, Issue 1.

The Analytical Committee works closely with other IATDMCT scientific committees, particularly the Anti-Infective Drugs Committee, and provides cross-disciplinary expertise on analytical method development and validation. Potential joint initiatives with the Clinical Toxicology & Drugs of Misuse, Toxicology & Environmental Health, and Pharmacometrics Committees are being explored.

The Committee supports cooperation with IATDMCT regional sections, including the Regional Asia-Pacific Section (RAPS) and initiatives to expand analytical capacity and TDM services in Indonesia and other resource-limited settings.
